TLDR:

Horde: just behind and slight left of Forgemaster Zak'aal on the Terrace of Crafters
Alliance: opposite corner of the Boralus Blacksmithing pit to the stairs

Another relatively quick achievement given I had max level Zandalari Blacksmithing and had no catch up needed on that, and I had all the ore and expulsum I needed. I followed Zygor (in-game addon) for the quick questline for the recipe and then headed to the two dungeons to get the specialty materials.
This Wowhead post helped clarify things for me. After scrolling through the complaints about the utility of the hammer I found Perculia's post (118) outlining what you need to craft it.
I ran the full Motherlode Dungeon on Mythic (because I had never done so on this toon) and got a big chunk of the Titan's Blood needed as well as Azerokk's Fist. I reset and ran up to Azerokk on normal 2-3 more times to get the remainder of the blood. These drop of the Shalebiters and Stonefury on the slope just before Azerokk - only about 8-10 mobs.
Then over to Stormsong Valley to Shrine of the Storms. Horde have a handy flight path right there. Kill Tidesage Council for the Precision Blessing Tools. (If you decide to continue on and swim through to final boss, stick near the bottom of the tunnel and don't get hit by the lightning jellyfish.)
Heading back to the Terrace of the Crafters in Dazar'Alor you need to use the Khaz'gorian Smithing Anvil (that you made back in Deepholm during the questline) to make the hammer. This is a golden topped anvil just behind Forgemaster Zak'aal.
